Howard Creek Bridge 496 is located at Mile Post 9.0 on Index-Galena Road where it crosses Howard Creek south of the North Fork Skykomish River in the Mount Baker-Snoqualmie National Forest.

The bridge was a single span, 61-foot long structure built in 1976. The log girders were deteriorating and traffic was restricted to one lane, with a 5-ton weight limit since August, 2008.

The deficient bridge was replaced with an 80-foot long single-span bridge with steel girders and a concrete deck. It is 28-feet wide and now facilitates two lanes of traffic.
